Novel biomarker for precision medicine

Biomarker CDx project aims to develop one novel predictive biomarker detectable through immunohistochemistry, for its use to identify cancer patients with a TGFβ-activated microenvironment in colorectal cancer (CRC) and other tumor types. Our biomarker is patent protected.

CRC is the third most-diagnosed cancer in EU and USA. With more than one million new cases of CRC diagnosed per year, the possibility of sparing unnecessary chemotherapeutic treatments would represent substantial savings for health care systems and will prevent unnecessary suffering to patients. Current clinical guidelines fail to prescribe the right treatment for an elevated number of patients which eventually return to the clinic after treatment, because of a reappearance of the disease. Thus, accurate methods to identify those patients at high risk of relapse after surgery are of vital importance for treatment selection. Pharmaceutical companies are aware of the need of placing safer and more effective medicines into the market and they are in need of tests to select patients who will benefit from these drugs. These tests are known as companion diagnostic tests (CDx) and are used in personalized medicine. Our biomarker identifies tumors with a TGF-beta activated stroma. We have shown that these tumors respond to anti TGF-beta therapy in experimental models of advanced CRC cancer, by blocking metastasis formation. Therefore, our biomarker can be used as a CDx for TGFβ inhibitors, this new class of therapies that have burst into the pipelines of many pharmaceutical companies in the last years for cancer treatment. Immunotherapies, which are very promising novel therapies, only work in a fraction of patients (<5% for CRC), leaving most patients with an urgent unmet medical need. Our group and others have recently shown that a percentage of patients irresponsive to immunotherapies could become responsive when receiving a combination of the immune checkpoint inhibitor with a TGFβ inhibitor (Tauriello et al., Nature 2018; Mariathasan et al., Nature 2018). Our biomarker identifies those patients which are unlikely to respond to immune checkpoint therapies alone, unless combined with TGFβ inhibitors.

In summary, our biomarker can be used for personalized medicine purposes by selecting those patients able to respond to TGFβ inhibitors and patients non-responders to immune checkpoint inhibitors. It could be also used as a prognostic method to better classify the aggressiveness of CRC and to identify patients at risk of recurrence and metastasis. Finally, our biomarker is a simple method what could facilitate the transition from current clinical practice to precision medicine, making it attractive to many companies developing TGF-beta inhibitors.

El projecte Biomarker CDx pretén desenvolupar un nou biomarcador predictiu detectable mitjançant immunohistoquímica, per al seu ús per identificar pacients amb càncer amb un microambient activat per TGFβ en càncer colorectal (CRC) i altres tipus de tumors. El nostre biomarcador està protegit per patent.

CRC és el tercer càncer més diagnosticat a la UE i als Estats Units. Amb més d’un milió de nous casos de CRC diagnosticats a l’any, la possibilitat d’evitar els tractaments quimioterapèutics innecessaris representaria un estalvi substancial per als sistemes d’atenció sanitària i evitarà patiments innecessaris als pacients. Les directrius clíniques actuals no prescriuen el tractament adequat per a un nombre elevat de pacients que eventualment tornen a la clínica després del tractament, a causa d’una reaparició de la malaltia. Així, els mètodes precisos per identificar aquells pacients amb alt risc de recaiguda després de la cirurgia són de vital importància per a la selecció del tractament. Les empreses farmacèutiques coneixen la necessitat de posar al mercat medicaments més segurs i eficaços i necessiten proves per seleccionar els pacients que es beneficiaran d’aquests medicaments. Aquestes proves es coneixen com a proves diagnòstiques d’acompanyament (CDx) i s’utilitzen en medicina personalitzada. El nostre biomarcador identifica tumors amb un estroma activat per TGF-beta. Hem demostrat que aquests tumors responen a la teràpia anti-TGF-beta en models experimentals de càncer de colon avançat, bloquejant la formació de metàstasi. Per tant, el nostre biomarcador es pot utilitzar com a CDx per als inhibidors del TGFβ, una nova classe de teràpies que han irromput en les pipelines de moltes empreses farmacèutiques en els darrers anys per al tractament del càncer. Les immunoteràpies, que són teràpies molt prometedores, només funcionen en una fracció de pacients (<5% per CRC), deixant la majoria de pacients amb una necessitat mèdica urgent no coberta. El nostre grup i altres han demostrat recentment que un percentatge de pacients que no responen a les immunoteràpies podrien respondre en rebre una combinació d’un inhibidor del punt de control immunitari amb un inhibidor del TGFβ (Tauriello et al., Nature 2018; Mariathasan et al., Nature 2018). El nostre biomarcador identifica aquells pacients que és poc probable que responguin només a inhibidors del punt de control immunitari, tret que estiguin combinades amb inhibidors del TGFβ.

En resum, el nostre biomarcador es pot utilitzar per a medicina personalitzada seleccionant aquells pacients capaços de respondre als inhibidors del TGFβ i identificant els pacients que no respondrien als inhibidors del punt de control immunitari. També es pot utilitzar com a mètode pronòstic per classificar millor l’agressivitat del CRC i identificar pacients amb risc de recurrència i metàstasi. Finalment, el nostre biomarcador és un mètode senzill que pot facilitar la transició de la pràctica clínica actual a la medicina de precisió, fent que sigui atractiu per a moltes empreses que desenvolupen inhibidors de TGF-beta.
