First Specific Non-Peptide Inhibitors of the Calcineurin-NFATc Signalling Pathway

Organ transplantation has emerged as an effective method for the treatment of end-stage organ failure and accounts for 120,000 people worldwide in 2014. In particular, vascularized pancreas transplantation is the only end treatment in type 1 diabetic patients (estimated 1.5 % of US population). The global transplantation market size was around US$5.1bn by 2014. North America led the global transplant immunosuppressant drugs market with a share of 46.2%, Europe is the second. Nowadays, the peptide drugs cyclosporin A (CsA) and Tacrolimus (or FK506) are the cornerstone of transplant immunosuppressive therapy. The target of both drugs is calcineurin (CN) phosphatase. Both drugs inhibit dephosphorylation of all CN substrates. As a consequence, despite their benefits, these drugs provoke severe side effects such as nephrotoxicity, neurotoxicity, gastrointestinal disturbances, diabetes, hyperlipidemia/hypercholesterolemia, among others. These effects have been linked to their mechanism of action and lifetime administration. Nowadays, a hallmark in the immunosuppressive field is to find better CN inhibitory compounds that specifically inhibit CN-NFATc signaling without blocking CN phosphatase activity, which is crucial to reduce the severe side-effects of current immunosuppressive drugs CsA and FK506. We discovered several ligands (patent application: EP15382332.3 with priority date 22/06/2015) which are the first, non-peptidic, small compounds that strongly and selectively inhibit dephosphorylation of the transcription factors NFATc by CN and thus NFATc signaling, T cell activation and the immune response. These ligands compete with NFATc binding to CN (activity in the low micromolar range) and do not affect to general phosphatase activity of CN. Therefore, lesser side effects than current drugs are expected. Thus these liganfs could markedly improve patient quality of life and reduce Health care costs. Oral administration is feasible and no cytotoxicity has been observed. In this context, the specific objectives of this project are a fat hit-to-lead process to identify low nM activity compounds that inhibit CN-NFATc signaling and perform pre-clinical mice experiments.

PRIMERS INHIBIDORS NO PEPTÍDICS ESPECÍFICS DE LA SENYALITZACIÓ DE CALCINEURINA-NFATC

El trasplantament d’òrgans s’ha convertit en un mètode eficaç per al tractament de la insuficiència orgànica terminal. El 2014 es s’han fer 120.000.a tot el món. En particular, el trasplantament de pàncrees vascularitzat és el tractament final en pacients diabètics tipus 1 (1.5% aproximadament de la població dels Estats Units). El mercat mundial del trasplantament va ser d’aproximadament 5.1billions de dòlars en el 2014. Amèrica del Nord va liderà el mercat mundial dels fàrmacs immunosupressors amb una quota del 46,2%, a continuació hi era Europa. Avui dia, els fàrmacs peptídics ciclosporina A (CsA) i tacrolimus (FK506 o) són la pedra angular de la teràpia immunosupressora del trasplantament. La diana d’ambdós fàrmacs és la fosfatasa calcineurina (CN). Tots dos fàrmacs inhibeixen la desfosforilació de CN sobre tots els substrats. En conseqüència, malgrat els seus beneficis, aquests fàrmacs provoquen greus efectes secundaris com nefrotoxicitat, neurotoxicitat, trastorns gastrointestinals, diabetis, hiperlipidèmia / hipercolesterolèmia, entre d’altres. Aquests efectes secundaris estan relacionats amb als seu mecanisme d’acció i la seva administració de llarga durada. Avui en dia, un repte crucial en el tractament immunosupressor és desenvolupar compostos inhibidors específics de l’activitat NFATc que inhibeixen específicament la senyalització CN-NFATc sense bloquejar l’activitat de la fosfatasa CN, fet què és crucial per reduir els greus efectes secundaris dels fàrmacs immunosupressors actuals CsA i FK506. Nosaltres hem identificat els primers lligands no peptídics (sol·licitud de patent: EP15382332.3 amb data de prioritat 2015.06.22), de baix pes molecular, que inhibeixen marcadament i de forma específica la senyalització de CN-NFATc en cèl·lules T activades i la resposta immune. Aquests lligands competeixen pel lloc d’unió amb CN i d’aquesta forma inhibeixen la desfosforilació dels factors de transcripció NFATc per Cn i la seva posterior senyalització cel·lular. Per tant, no inhibeixen l’activitat fosfatasa general de CN. En conseqüència, es d’esperar que aquests lligands presentin menys efectes secundaris que els fàrmacs actuals, el que podria millorar notablement la qualitat de vida del pacient i reduir els costos del Sistema Nacional de Salut. A més, l’administració oral és factible i no s’ha observat citotoxicitat. En aquest context, els objectius específics d’aquest projecte són identificar compostos amb activitat inhibidora dels NFATc a nivell de nM i realitzar experiments preclínics en ratolins.
