Combined CD6 therapy for the treatment of

infectious diseases and related inflammatory processes

Sepsis is a life-threatening condition caused by the host response to an infectious agent, most commonly bacterial, that affects 3 in 1000 people in developed countries and the cause of almost 40,000 deaths per year in Spain alone. Sepsis is a dysregulated systemic inflammatory response syndrome (SIRS) caused by an infection, leading to an overwhelming and sustained pro-inflammatory state and death via multi-organ dysfunction syndrome (MODS) and cardiovascular collapse (septic shock). Antibiotics alone do not contribute to reduce MODS and/or SIRS associated mortality, more so in an increasingly Multidrug Resistant (MDR) bacterial environment. It is thus mandatory to search for alternative non-antibiotic (adjunctive) strategies that target both bacterial virulence factors and host inflammatory and immune mediators.

The CD6 glycoprotein is a lymphocyte surface receptor that enables adhesive cell-to-cell contacts with antigen presenting cells to modulate activation/differentiation processes and regulation of T-cell development. Dr. Lozano’s team has unveiled the bacterial binding properties of CD6 and reported that the soluble extracellular region of CD6 binds to and aggregates G- and G+ bacteria by strongly interacting with bacterial endo- and exo-toxins. Based on this property, the group further demonstrated that infusion of a recombinant soluble form of human CD6 (rshCD6, devoid of TM region and cytoplasmic tail), protects from SIRS and MODS induced death and reduces serum levels of pro-inflammatory cytokines (IL-1β, IL-6 and TNF-α) in experimental mouse models involving G+ and G- bacteria irrespective of MDR phenotype. Dr. Lozano’s results show time- and dose-dependent effects of rshCD6 infusion in mice survival post SIRS and MODS and shows improved effects when combined with the broad-spectrum bactericidal antibiotic Imipenem (but not other antibiotics such as antibiotics of the same or different family, e.g., Meropenem or Erythromycin). Therefore, the combination therapy of CD6 and Imipenem has therapeutic potential for the intervention of septic shock syndrome and other inflammatory diseases, providing a novel composition comprising a CD6 derived product and Imipenem. La sèpsia és una condició mortal causada per la resposta de l’hoste a un agent infecciós, més freqüentment bacterià, que afecta a 3 de cada 1000 persones a països desenvolupats i que provoca gairebé 40.000 morts anuals només a Espanya. La sèpsia és una síndrome de resposta inflamatòria sistèmica no regulada (SIRS) causada per una infecció, que condueix a un estat pro-inflamatori aclaparador i sostingut, a través de la síndrome de disfunció multi-orgànica (MODS) i el col·lapse cardiovascular (xoc sèptic). Els antibiòtics sols no contribueixen a reduir la mortalitat associada a MODS i /o SIRS, més encara en un entorn bacteriològic creixent de multi-resistències antibiòtiques (MDR). Per tant, és obligatori buscar estratègies alternatives no-antibiòtiques (adjuntives) que s’orientin tant contra factors de virulència bacteriana com contra els mediadors inflamatoris i immunitaris de l’hoste.

La glicoproteïna CD6 és un receptor de superfície limfocitària que permet contactes cel·lulars adhesius amb cèl·lules presentadores d’antígens i modula processos d’activació/diferenciació i regulació del desenvolupament de cèl·lules T. L’equip del Dr. Lozano ha descrit les propietats d’unió entre bacteris i la molècula CD6 i ha caracteritzat la forta unió entre la regió extracel·lular soluble de CD6 i bacteris tant Gram positius com negatius mitjançant llur endo- i exo-toxines. Basant-se en aquesta propietat, el grup també ha demostrat que la infusió d’una forma soluble recombinant de CD6 humà (rshCD6, sense regió TM ni cua citoplàsmica), protegeix contra la mort induïda per MODS i SIRS tot reduint els nivells sèrics de citocines pro-inflamatòries (IL- 1β, IL-6 i TNF-α) en models experimentals de ratolí amb bacteris G+ i G-, independentment del seu fenotip MDR. Els resultats del Dr. Lozano mostren també efectes sobre la supervivència dependents de temps i dosi en la infusió de rshCD6 en ratolins SIRS i MODS i es mostren efectes additius quan es combina amb l’antibiòtic bactericida Imipenem d’ampli espectre (però no amb altres antibiòtics de la mateixa o diferent família com ara, Meropenem o Eritromicina). Per tant, la teràpia combinada de rshCD6 i Imipenem té potencial terapèutic per a la intervenció de la síndrome de xoc sèptic i altres malalties inflamatòries, proporcionant una nova composició que inclou un producte derivat de CD6 i Imipenem.
